Common Misconceptions About Psychotherapy


Some ideas about therapy show up so often in fiction I find myself wondering how many writers are using them deliberately and how many just don't realize they're inaccurate. Here are six of the most common, along with some information on more standard current practice.

1. You lie on a couch

Reality: Therapy clients don't lie on a couch; some therapists' offices don't even have couches.

So where did this come from? Sigmund Freud had his patients lie on a couch so he could sit in a chair behind their heads. Why? No deep psychological reason -- he just didn't like people looking at him.

There are a lot of reasons modern therapy clients wouldn't be happy with this. Imagine telling someone about difficult or embarrassing experiences and not only not being able to see them, but having them react with silence. Why on earth would you want to go back?

The ideal therapeutic setup, and they actually teach this in graduate school, is to have both chairs turned inward at about a 20 degree angle(give or take about 10 degrees), usually with 8 or 10 feet between them. Often the therapist and the client end up facing each other because they turn toward each other in their chairs, but with this setup the client doesn't feel like s/he's being confronted.

Even if there is a couch in the room, the therapist's chair will almost invariably be turned at an angle to it.

2. Therapists analyze everyone

Reality: Therapists don't analyze people any more than the average person, and sometimes less often.

Ironically, only people trained in Freud's make-the-patient-lie-on-the-couch-and-free-associate-about-Mother approach (aka psychoanalysis) are taught to analyze at all. All other therapists are taught to understand why people do things, but it takes a lot of energy to figure people out. And to be very frank, while therapists are usually caring folks who want to help their clients, in day-to-day life they're dealing with their own issues and don't necessarily have the time or space to care about everyone else's problems or behaviors.

And the last thing most therapists want to hear about in their spare time is strangers' problems. Therapists get paid to deal with other people's problems for a reason!

3. Therapists have sex with their clients

Reality: Therapists never, ever, ever have sex with their clients, or the friends or family members of clients, if they want to keep their licenses.

That includes sex therapists. Sex therapists don't watch their clients have sex, or ask them to experiment in the office. Sex therapy is often about educating and addressing relationship problems, since those are two of the most common reasons people have sexual problems.

Therapists aren't supposed to have sex with former clients, either. The rule is that if two years have passed and the former client and therapist run into each other and somehow hit it off (ie this wasn't planned), the therapist won't be thrown out of professional organizations and have licenses revoked. But in most cases other therapists will still see them as suspect.

The reasoning behind this is simple -- therapists are to listen and help without involving their own issues or needs, which creates a power differential that's difficult to overcome.

And truth be told, the roles therapists play in their offices are only facets of who they really are. Therapists focus all of their attention on clients without ever complaining about their own concerns or insecurities.

When people think they want to be friends, they usually want to be friends with the therapist, not the person, and a true friendship involves sharing power, and flaws, and taking care of each other to some extent. Getting to know a therapist as a real person can be disenchanting, because now they want to talk about themselves and their own issues!

4. It's all about your mother (or childhood, or past...)

Reality: One branch of psychotherapeutic theory focuses on childhood and the unconscious. The rest don't.

Psychodynamic theory kept Freud's psychoanalytic belief that early childhood and unconscious mechanisms are important to later problems, but most modern practitioners know that we're exposed to a lot of influences in day-to-day life that are just as important.

Some therapists will flat-out tell you your past isn't important if it's not directly relevant to the current problem. Some believe extensive discussion of the past is an attempt to escape responsibility (Gestalt therapy) or keep from actively working to change (some types of cognitive-behavioral theory). Some believe that the social and cultural environments we live in today are what cause problems (systems, feminist, and multicultural therapies).

5. ECT is painful and used to punish bad patients

Reality: Electro-convulsive treatment (in the past, called electro-shock treatment) is a rare, last-resort treatment for clients who have been in and out of the hospital for suicidality, and for whom more traditional treatments, like medications, haven't worked. In some cases, the client is so depressed she can't do the work to get better until her brain chemistry is working more effectively.

By the time ECT is a consideration, some clients are eager to try it. They've tried everything else and just want to feel better. When death feels like your only other option, having someone run a painless current through your brain while you're asleep doesn't sound like such a bad idea.

ECT is not painful, nor do you jitter or shake. Patients are given a muscle relaxant, and because it's frightening to feel paralyzed, they're also briefly placed under general anesthesia. Electrodes are usually attached to only one side of the head, and the current is introduced in short pulses, causing a grand mal seizure. Doctors monitor the electrical activity on a screen.

The seizure makes the brain produce and use serotonin, norepinephrine, and dopamine, all brain chemicals that are low when someone is depressed. Some people wake up feeling like a miracle has occurred. Several sessions are usually required to maintain the changes, and then the individual can be switched to antidepressants and/or other medications.

ECT is no more dangerous than any other procedure administered under general anesthesia, and many of the potential side effects (confusion, memory disturbance, nausea) may be as much a result of the anesthesia as the treatment itself.

6. "Schizophrenia" is the same thing as having "multiple personalities"

Reality: Schizophrenia is a biological disorder with a genetic basis. It usually causes hallucinations and/or delusions (strong ideas that go against cultural norms and are not supported by reality), along with a deterioration in normal day-to-day functioning. Some people with schizophrenia become periodically catatonic, have paranoid thoughts, or behave in a disorganized manner. They may speak strangely, becoming tangential (wandering verbally, often in a way that doesn't make sense to the listener) using nelogisms (made up words), clang associations (rhyming) or, in extreme cases, producing word salads (sentences that sound like a bunch of jumbled words and may or may not be grammatically correct).

Dissociative Identity Disorder (formerly multiple personality disorder) is caused by trauma. In some abusive situations, the normal defense mechanism of dissociation may be used to "split off" memories of trauma. In DID, the split also includes the part of the "core" personality attached to that memory or series of memories. The dissociated identity often has its own name, traits, and quirks; and may or may not age at the same rate as the rest of the personality (or personalities), if it ages at all.

Therefore, referring to oneself as "schizo" or "schizoid" or "schizophrenic" when one means one has an alter ego or contradictory personality traits makes no sense (and is guaranteed to make the psychologically savvy wince)!

Secrets to Successful Brewing

My main goal here at Cryptobrewology is to introduce people to the wonderful world of craft beers and home brewing, so I'd be remiss if I didn't explore the possibilities even for those who just want to give home brewing a try, and don't necessarily have the intention of taking it to the next level. In this case, I'm talking about the hobbyist who might give a Mr. Beer home brewing kit a whirl, just for fun.

In many conversations that I've overheard, or have been a part of regarding Mr. Beer, the comments are pretty much the same, "I tried Mr. Beer once, but it tasted bad."

Typically, when people give something a try and it doesn't turn out as expected, they are all too willing to blame the product or the system they used, and not evaluate their methods.

Mr. Beer sometimes gets a bad rap. Prior to sitting down to write this I actually tasted a Mr. Beer West Coast Pale Ale I had brewed, and it tasted great. Not yeasty, not sour, or sweet. It was mildly bitter, with a fruity and mildly floral hop nose. It had all the hallmarks of a commercial ale that I may have bought at the store. No kidding.

The key factors that led to this great tasting batch of Mr. Beer are proper sanitization, yeast pitching temp, fermentation temp, water quality, and patience. Ignore these and you're destined to make a lousy brew, so don't blame Mr. Beer!

Key Factor One: SANITIZE

I can't stress it enough. Sanitize, Sanitize, SANITIZE! Make sure your kitchen counters are clean, make sure your hands are clean, and make sure you use the One Step no-rinse cleanser - included with every kit and refill - to sanitize the keg and your tools. Before bottling make sure you follow the directions and thoroughly sanitize the bottles.

No rinse means "NO RINSE!" There is absolutely nothing left behind by One-Step that can harm you or the beer. Rinse with tap water and you risk contaminating everything you have just cleaned. Don't rinse, just drain.

Key Factor Two: PITCHING and FERMENTING TEMP

The Mr. Beer instructions advise us to use cold water in the fermenter prior to pouring in the wort, and then topping off with cold water before pitching the yeast. There is a reason for this. First, the wort is very hot and you don't want to compromise the plastic keg fermenter, so put cold water in first, as a buffer.

Second, the yeast should not be added to the keg until the temperature is just about 70 degrees fahrenheit. 85 is not just about 70. You can monitor the temperature with a SANITIZED Thermometer, that's what I do, or buy a Brew-O-Meter from Mr. Beer that sticks onto the fermenter, that works.

Topping off with cold water brings the brew up to the full fermenting volume, and will also help drop the temperature of the wort to within pitching range. If you don't have a thermometer, just feel the sides of the keg. If it is cool to the touch, not warm or hot, then you're okay. If the keg feels warm to you it's too warm for the yeast!

After pitching the yeast you want to keep the keg somewhere out of direct sunlight, and where the temperature is consistent, somewhere between 68 and 72 degrees fahrenheit is recommended.

Temperature is critical for proper yeast performance. Too hot or too cold and you can kiss your beer goodbye.

Key Factor Three: WATER

Bottled water or filtered tap water are recommended for a reason. They are filtered to remove contaminants and will ensure a decent quality beer. I watched in horror as a YouTuber demonstrating Mr. Beer showed me how to top off the keg with cold water using his sink sprayer! You're just asking for trouble if you do that. Who knows what microbial monstrosities are hanging out in the end of that thing?!

It's better to just buy a few gallons of bottled water and stick them in the fridge. Case closed.

Key Factor Four: PATIENCE

Sanitization, temperature and water quality are critical components to producing a good batch of Mr. Beer - well, ANY home brewed beer actually - but another important factor, something many of us DIY-ers struggle with, is patience.

Sure, you can have drinkable beer in as little as two weeks with any home brewing process. That's because, generally speaking, the fermentation process is usually complete in 7 days, and it only takes 7 days for the beer to carbonate after bottling. It's drinkable, but it can be a lot better.

What makes it better is aging. Beer left alone in the fermenter for two weeks will settle and clear out a lot. There will be less sediment in the bottles, so less muck to stir up while pouring.

Remember that beer I mentioned? Do you know how long it was in the bottle? I waited a tad over two weeks before bottling, so the beer had cleared out considerably in the keg. Once bottled, I waited one week before putting the bottles into the refrigerator to halt carbonation*. I tried one beer each week after that.

The first one was a little "bready." That's one complaint I often hear about Mr. Beer. The yeasty odor is common early after bottling in any home brewed beer.

The second beer was less yeasty, but it had a slightly sour character.

The third beer, my most recent, had been in the bottle for about three weeks. That's FIVE weeks since the day I brewed it.

Patience is a must! Don't pour out your beers after only giving them a couple of weeks to mature. Commercial craft brewers let their beers rest, or age, in the bottles for three or four weeks before they hit the market. The full flavor of the beer needs that much time to develop.

To summarize, don't knock Mr. Beer. You can get good results brewing your own beer. Brew it right, brew it clean, and be patient. You'll find that the product isn't the problem, it's the brewer's technique and patience.

* I read somewhere that it's a good idea to chill the beer to halt carbonation, but if the beer has fermented fully before adding priming sugar and bottling, the yeast should have only enough to carbonate and will stop on its own. Also, this trick might work for ale yeast but lager yeast will continue to ferment at lower temps if sugar is present.

Patients are always surprised to learn that there are very few laws governing the practice of medicine. In fact, a physician who is licensed to practice medicine by his state medical board - whatever his specialty - can legally provide counseling for anyone, even if he or she has absolutely no training in psychology at all!

For instance, just because a doctor calls himself a psychiatrist is no guarantee that he is actually competent to practice psychiatry. For example, legally, a proctologist, a medical doctor certified as a specialist only in disorders of the rectum, can label himself as a non-board certified psychiatrist and do adolescent counseling for drug problems or psychotherapy for suicidal patients-all, with no training in psychology.

A urologist, certified only as a specialist in treating urinary disorders, can again identify himself or herself as a psychiatrist, though non board-certified, and still treat a dangerously bipolar patient; and a dermatologist, actually certified as a specialist only in skin disorders could call himself a psychiatrist and treat your child for ADHD-it is all legal and can be done with no training whatever in psychology or mental health treatment.

These three examples may seem ludicrous. But, remember men and women often get their treatments for depression and anxiety without ever checking, or even caring to see if the medical doctor they are consulting is board-certified in psychiatry?

Do not assume that your medical doctor has the right credentials to perform high quality psychological, counseling or psychotherapeutic treatments. After all, even board certification in psychiatry is no guarantee because most board-certified psychiatrists have only 3 years of specialized training in mental health treatment.

Most of their training is in physiology, chemistry, sciences and general medicine. Therefore, they usually prescribe drugs and do little, if any, counseling or psychotherapy.

Screen your possible therapist with the following 3 questions:

1. Are you certified by the American Board of Psychiatry and Neurology?

Make certain that if your doctor labels himself as a psychiatrist that he is certified by the American Board of Psychiatry and Neurology. If he is, that would mean that he has received specialized professional training in providing mental health treatment.

However, remember that the psychiatrists training often has been centered on physical therapies, like the prescribing of drugs and even the use of shock treatments. Psychiatrists sometimes receive only little training in psychotherapy, counseling or psychological testing.

You know what that means. If you consult a psychiatrist the odds are that you will only walk out with a prescription.

Certainly, there are some psychiatrists who are better-rounded in their training and who do some psychotherapy. Nevertheless, odds are that if your doctor has an MD or DO degree, you will walk out with a prescription as your main or only treatment.

2. Are you certified or licensed by the state Board of Psychology as a clinical psychologist?

While there are many thousands of professionals in the United States who do some type of counseling or psychotherapy, only a small percentage of these are the elite practitioners who are licensed to practice clinical psychology!

To become a clinical psychologist requires approximately 6 years of specialized college and graduate training in mental health, counseling, psychotherapy and diagnostic testing. This training is obtained during the last two years of college, followed by approximately 3 to 4 years of specialized graduate school after college.

The psychologist has a doctoral degree in psychology-the study of emotions, thinking and behavior. Psychologists then must undergo the equivalent of a one year internship in clinical psychology- no irrelevant courses in proctology or anatomy, just massive amounts of training in psychological treatment, psychotherapy, family therapy, counseling and specialized diagnostic testing.

Only four years of medical school with training in anatomy, obstetrics and other medical sciences in addition to perhaps a few weekend courses or casual reading about psychology, may be the only education that your non-certified psychiatrist may have. This is certainly no substitute for the psychologists rigorous and highly specialized training over many years.

Remember, you are protected when you consult a clinical psychologist. Why? Because any professional who calls himself a clinical psychologist and is licensed to practice clinical psychology independently, has at least 6 to 7 years of college and graduate school training specifically targeted to psychology.

Unless you check the board certification of the medical doctor you are consulting, however, you may not discover that you have been consulting someone who may have little or even no training in mental health and psychological treatment. In fact, you may discover that you have been receiving dangerously inferior forms of psychiatric care, without ever realizing it.

3. Is the state board of psychology or medicine investigating you for any complaints or malpractice suits?

It is true that even great psychiatrists and clinical psychologists do occasionally get sued. However, you can contact your states professional board to learn whether your therapist has demonstrated a pattern of poor care or inappropriate behavior.

Questions that you are commonly told to ask but which are not relevant:

1. To those calling themselves psychiatrists: Are you board certified?

Nearly all medical doctors are certified by one board or another. If a person has an MD or DO degree, just being board certified in anything is not good enough either. The right question to ask is, Are you certified by the American Board of Psychiatry and Neurology?

2. In what state are you licensed to practice psychotherapy and counseling?

Remember states do not specifically license medical doctors to practice psychotherapy and counseling. Absolutely any physician with a valid medical license can legally provide these specialized mental health services-even if they have no psychiatric training at all!

There are no regulations that limit medical doctors to practice within their specialty; not one of the 50 states forbids the practice of psychiatry by non-psychiatrists. The correct question to ask a medical doctor is, Did you complete a formal residency in psychiatry?

3. If the therapist is not a medical doctor, but a clinical psychologist, you are better protected. All you need to ask is, Are you licensed by the state to practice clinical psychology independently?
